Wegovy · Starting out
Your first week on Wegovy
Published by MALAI · Last updated
Week one is 0.25 mg — the same molecule and the same starting dose as Ozempic. What differs is where it goes: Wegovy climbs through five steps to a 2.4 mg ceiling over sixteen weeks, twice the length of Ozempic's titration. Week one feels identical; the path afterwards does not.
What happens, day by day
Day 1
Inject and expect nothing dramatic. The starting dose is deliberately below the level that drives appetite suppression.
Days 1–3
If side effects arrive, this is when. Note that the 44% nausea figure quoted for Wegovy is the rate across the full trial at 2.4 mg, not the week-one rate — at 0.25 mg you are far below that.
Days 4–7
Appetite starts to change. Because the climb ahead is long, week one is worth using to build the eating pattern you will need at 2.4 mg, rather than treating it as a quiet period.
End of week 1
Little to no weight change. The 16-week titration means the first month is about tolerance, not results.
Where 0.25 mg sits in the ladder
0.25 mg → 0.5 mg → 1 mg → 1.7 mg → 2.4 mg across sixteen weeks. By week 4 you are moving to 0.5 mg, with three escalations still ahead. Each step can re-trigger nausea that had settled, which is the part people are least warned about — week one is not the hardest week; week nine often is.
What to eat in week one
Wegovy is the same molecule as Ozempic, so the timing works the same way: the peak lands 1 to 3 days after your injection, not on the day (prescribing information, section 12.3). What differs is how long you will be doing this. Your climb runs sixteen weeks through five doses, and every step up can restart gut symptoms that had already settled — which means week one is not a week to get through, it is a week to build something you can still hold at 2.4 mg in four months. Two things carry that far. Protein at 1.4 to 1.6 g per kg a day, and a fourth eating occasion somewhere around 3 or 4 PM. Almost everyone loses protein by quietly dropping that afternoon slot, and they drop it around month two when appetite really falls — so make it a fixture now, while you still want it.
What Wegovy is licensed for — and why that matters now
Wegovy is FDA-approved specifically for chronic weight management, approved in 2021, for adults with BMI 30+ or 27+ with a weight-related condition, and for adolescents 12 and over. That approval is the practical difference from Ozempic: because weight management is the licensed indication, insurance coverage is possible rather than automatically excluded. It is still frequently denied or requires prior authorisation, but there is a route.

What is normal, and what is worth a call
Expected: nausea that eases within days, early fullness, some constipation. Call your prescriber for persistent vomiting, severe abdominal pain, or symptoms of gallbladder trouble — rapid weight loss raises that risk independently of the drug.

Frequently asked questions
- Is the first week on Wegovy the same as Ozempic?
- Physically, yes — same molecule, same 0.25 mg starting dose, same adaptation period. The difference is the trajectory. Ozempic reaches its 2 mg ceiling in roughly eight weeks; Wegovy climbs through five steps to 2.4 mg over sixteen. You are starting the same, but committing to twice the titration.
- Is 44% nausea what I should expect in week one on Wegovy?
- No. That 44% is the rate across Wegovy's full trials at the 2.4 mg maintenance dose against 16% on placebo, per the prescribing information section 6.1 — not a week-one figure. Week one is 0.25 mg, roughly a tenth of that dose. Quoting the trial-wide number as a first-week expectation is one of the more common errors in GLP-1 content.
- I have had no side effects at all on Wegovy — is it actually working?
- Almost certainly. The 44% nausea figure from Wegovy's trials is the rate at the 2.4 mg maintenance dose against 16% on placebo (prescribing information, section 6.1), which also means 56% of people on the full dose never reported it. You are on 0.25 mg, roughly a tenth of that. Side effects are not a progress bar: they track how fast your gut is adapting to a dose, not how well the drug works. Watch whether portions you finished last month now feel like too much.
- What should I eat on the day of my Wegovy injection?
- Eat normally, and save the caution for day two. The advice to eat light on shot day is well meant but mistimed: semaglutide reaches peak concentration 1 to 3 days after the dose (prescribing information, section 12.3), so injection day is usually the most comfortable day of your week. Make days two and three the plain ones instead. Then use day one, while eating is still easy, to bank protein — 1.4 to 1.6 g per kg is far simpler to hit before appetite drops.
- Does hair thinning start in the first week on Wegovy?
- Common on Wegovy overall. Reported by 3% versus 1% on placebo in the label's trials. Tied to the pace of weight loss rather than the drug directly, and to the nutrient gaps that come with eating much less. In week one you are on 0.25 mg, below the doses those figures come from, so if it turns up at all it is usually milder than the label rate suggests.
